Short Term Disability (STD) FAQs & Resources
Union members who wish to withdraw from the union need to complete the Withdraw Authorization Form UM 70. The form must be dated between December 1 and 31 but can be submitted now. Once an employee completes the form, they need to submit it to Jodie Lewis for processing. Union dues will still be taken out of the January 14 paycheck, but removed from the January 28, 2026 paycheck.
HR Policy 5032 states that all MUHC Staff, Medical Staff, and MUHC Non-employees are required to get annual vaccination(s) as identified by the Infection Control Committee, and provide proof of vaccination/immunization to Employee Health by the communicated deadline or possess an approved medical or religious exemption.
