Union withdraw information and form

Service maintenance employees who wish to withdraw from the union can follow the steps outlined below to change their union status.

 

Key Points to Remember:

  • Form: Employees must complete the UM 70 Withdrawal of Authorization for Deduction of Organization Dues form.
  • Form Submission: Completed forms need to be sent to Jodie Lewis for processing. Do NOT send to UM MU Payroll.
  • Withdrawal period: Forms must be dated December 1-31 but can be submitted now.
  • Payroll impact: Dues will be deducted for December. The withdrawal becomes effective with the first payroll period beginning on or after January 1, 2026.
  • Final December payroll (union dues taken out): December 21, 2025 – January 3, 2026 (paycheck date January 14, 2026).
  • First payroll without dues: January 4-17, 2026 (paycheck date January 28, 2026).
  • Form Confirmation: Submitted forms will be confirmed by a response from Jodie Lewis upon receipt.
