If your existing phone is broken or not working you need to request to have it replaced. This is a billable item, which will be paid for by your department.
Request a new phone service, for user, department, fax line, alarm line or conference phone.
Request to modify the functionality of your phone. You can request shared lines to be added or removed and changes in other programming.
Request to have your phone service disconnected. There will be a charge if the number needs to be re-established in the future.
Request to have your phone moved to a different location. This is a billable item, which will be paid for by your department.
Request to add phone equipment such as a headset, side car or other to your existing phone.
