Learning Data Issue / Cleanup Request

Service Overview

This service allows users to report issues or inconsistencies in learning-related data and request corrections or cleanups. This includes duplicate records, incorrect assignments, outdated certifications, missing completions, or data mismatches between systems. The Learning Services team will investigate and resolve issues to maintain accurate records across systems such as SBX, HealthStream, and HR HUB.

Submit a request through the service catalog including the user(s) affected, the issue description, impacted system(s), and desired correction. Screenshots or report examples are encouraged for faster resolution.
