How to Change Your Name

  1. Click this link: https://tdx.umsystem.edu/TDClient/50/MUHCHR/Requests/ServiceDet?ID=1591
  2. Enter the following information into the text box from the link above.
    1. Current name
    2. Employee ID
    3. Previous Name
    4. Attach a copy of your updated social security card that lists your new name
  3. Ensure your information is updated in myHR. Visit myHR here and review "Documents" below for PDF guides.
  4. If applicable, please reach out to any certifying bodies to update any credentials. This will prevent issues upon renewal.
  5. Please monitor your AskHR ticket, looking for the "resolved" status. After your ticket is resolved, that is an indication that your new name information has been processed. Completing step four prior to processing will not reflect the name change.
  6. If your address has also changed, please ensure that information is updated with your new name.
  7. Select "Name/Credential/Title/Change" inside the ID Badge Request App and complete the prompted steps.
    1. Please complete all four sections of the Name/Credential/Title/Change option, listed at the top of the app page.

    2. Select "Name Change" under "Reason for Badge," < Reason for Request/Reason for Replacement.

  8. Please wait 14 business days for changes to be reflected. If a software is still displaying your previous name, please reach out to the IT Help Desk for assistance.
  9. Contact the IT Help Desk (573-882-5000) if you would like a change to your PawPrint.
