To Request Changes:
- Complete this form: Modify Phone. Please note: A manual approval of a valid MoCode by an authorized signer required. You may use the MoCode lookup tool located in the form
- Click the Submit button at the bottom of the form.
Types of Phone Service Changes (not limited to):
- Modifying the location of the phone
- Modifying the owner (assigned name) of a phone
- Removing or adding the appearance of an additional line (to answer/pickup)
- Setting up or updating the voicemail owner
If you are experiencing the following:
- No dial tone.
- Handset, phone, line cord, or handset cord is damaged.
- Calls are disconnected randomly or will not connect.
- Voicemail account is locked due to too many bad passwords, or delayed voicemails to your email inbox.
- Cannot login to or have problems with performance of a Cisco Finesse.
