Detailed Information

To Request Changes:

  1. Complete this form: Modify Phone. Please note: A manual approval of a valid MoCode by an authorized signer required. You may use the MoCode lookup tool located in the form
  2. Click the Submit button at the bottom of the form.

Types of Phone Service Changes (not limited to):

  • Modifying the location of the phone
  • Modifying the owner (assigned name) of a phone
  • Removing or adding the appearance of an additional line (to answer/pickup)
  • Setting up or updating the voicemail owner

If you are experiencing the following:

  • No dial tone.
  • Handset, phone, line cord, or handset cord is damaged.
  • Calls are disconnected randomly or will not connect.
  • Voicemail account is locked due to too many bad passwords, or delayed voicemails to your email inbox.
  • Cannot login to or have problems with performance of a Cisco Finesse.
