STEP 5 - Download to Excel

A listing of the FMLA requests reported to MetLife will load. Click the “Download” icon at the bottom left of the claims list. An Excel file containing all records will download for this leave time period. Use this report to review attendance/tardies in accordance with HR 5016. The program column “Federal FMLA” will list the dates and hours requested by the employee – review both the Decision and Reason columns when matching to UKG. The Leave Team will also use this information to update UKG, only noting dates with FMLA approval.

When reviewing – ask these questions:

Employees typically have five (5) calendar days to report their request to use FMLA to MetLife. MetLife has five (5) business days to review and make a decision.

Example: John reports an unplanned absence of 12 hours to leadership on Monday May 18 as intermittent FMLA following his normal call-in procedure. John also reports his absence of 12 hours to MetLife on Thursday May 21. MetLife renders a decision that appears on this report May 28 considering the weekend and Memorial day holiday.

The “reports to” leader listed in PS should receive notifications of new employee claims, including status updates/changes. Additionally, this leader should receive email notifications of intermittent absences approved as FMLA.
