Courtesy Appointment Request Process
The following steps outline how departments or hospital sponsors should request a courtesy appointment:
- The department or hospital sponsor must complete the Vetting Request Form as the requestor.
- In the 'New Request' section of the form, enter the relevant information for the employee being onboarded.
- Once the form is submitted, the Shared Services team will initiate Adobe Sign onboarding packets and send them to the employee.
- The employee will receive three separate onboarding packets via email.

- Staff Health Packet
- Employees must complete all packets to finalize their onboarding process.
External Vendor Process
The process for requesting a courtesy appointment for external vendors follows the same steps as above. Please ensure all vendor-related documentation is provided during the vetting request submission.

2. How long does it take to process a courtesy appointment request?
The process typically takes 10 business days, but may vary based on the volume of requests, required documentation, and completion time of candidate.
3. Can I submit a request for multiple employees at once?
No, each request must be submitted individually to ensure accurate processing and documentation.
4. Who should I contact if I encounter issues with the vetting form?

5. Do I need to make a staff health appointment?
Yes, the non-employee will need to make a staff health appointment upon completion of the staff health Adobe packet. To make a staff health appointment, please contact them by phone at 573-884-8770 or email umhshrstaffhealth@health.missouri.edu .
6. When will the courtesy appointment expire and how can it be extended, if needed?
All courtesy appointments except for credentialed clinicians have an expiration date of March 1 of the corresponding year. Hospital leaders do have the ability track the courtesy appointment termination dates via the Power BI Dashboard Courtesy Appointment Termination Tracker . If dates should be extended, an email should be sent to courtesyappointmentrequest@health.missouri.edu.
