Prior to submission of the fee adjustment form, please read the policies on the Office of the University Registrar’s website. Prior to submitting the form, you will be required to acknowledge that you have read all the policies.

Required Information and Attaching Documents

All required information marked with an asterisk must be completed prior to submission of the form. Attach supporting documentation (e.g., medical documentation, explanation of extenuating circumstances) in support of your fee appeal. See the link below for additional information on documentation.

Drop or Withdrawal Process

Prior to submission of the fee adjustment form, all course(s) listed on the fee adjustment form must be processed through the drop/withdrawal process.

Fee Adjustment Assessment

Any change in assessment can be adjusted or pro-rated only to a rate already established in the published university fee reassessment schedule (link below).
