Modify an Assigned Phone Number

How do I modify an assigned phone number?

To Reassign a Phone Number:

1. Complete this form: Modify Phone. Please note: A manual approval of a valid MoCode by an authorized signer required. You may use the MoCode lookup tool located in the form.
2. Click the Submit button at the bottom of the form.

Caution: For security reasons, voicemail accounts assigned to individual persons will be deleted upon transfer of phone number ownership. If you need to access unread voicemails in the mailbox, due to a personnel change, your department will need to request access before the transfer of ownership to a new person.

• Obtain the voicemail PIN from the prior owner and process the voicemails.
• If you cannot obtain the PIN, submit an electronic records request to gain access: https://www.umsystem.edu/ums/is/infosec/account-access
